Medicare Concierge
"Boutique style primary care"
This direct primary care membership is reserved for our Medicare patients and includes the same benefits as our DPC membership.
Medicare Concierge allows you to use your Medicare benefits with boutique-level access and care personalized to you.
We bill Medicare for covered services. Medicare and Medicare advantage plans work best with our service.
We are unable to accept Medicare with Medi-Cal due to state regulations.
Medical Weight Loss

We customize a weight loss program for your personal needs and goals. We can help you achieve this naturally or with assistance with medical weight loss therapy including advanced injectables and oral medications.
We start with a consultation to discuss your goals, lab tests (either covered by insurance or provided for a discount price), and an InBody scan.
Your plan will be tailored to your unique body and goals. Every plan includes access to prescription medication (medication cost not included; may bill insurance or self-pay), labs, and ongoing follow up to track progress (in person or telehealth option).
Lifestyle counseling including nutrition and exercise also included
Whether you choose medication-assisted or natural weight loss, we will support you every step of the way!

HRT may not be for everyone. The initial consultation and lab work is required to ensure that you'll benefit from medications.
How It Works
We start with a consultation, lab tests (either covered by insurance or provided for a discount price), an exam and an InBody scan.
Based on lab results we can create a hormone replacement therapy (HRT) plan specific for you. You can enroll in our all-inclusive program that includes medications (injectable testosterone only but topical may be prescribed and priced separately), occasional labs (every 3- 6 mo. Is $85 or covered by insurance), at-home self administered injections, and as-needed consultations (in person or telehealth).
If your insurance covers the medication we will prescribe it through your pharmacy.

We offer group discounts for small businesses.
Membership benefits for employees are identical to those offered to individuals and families.
Employers can customize an employee plan that covers individual only or include family members under the business account.

Membership Fees: A single membership costs $119 per month, while a family membership is priced at $300 monthly. Members can choose to pay annually to benefit from a 10% discount. There is a one-time registration fee of $150 per individual, with a maximum fee of $300 for families. All memberships require a minimum commitment of three months.

Adding Members: You can include additional adults or children in your plan if they live in the same household as the primary member.

Vaccination Services: Childhood vaccines are not offered at our clinic, but most local pharmacies can provide them.

Membership Details: Our membership plans are not insurance products and do not include care from unaffiliated providers. We suggest having at least a high-deductible insurance plan or a cost-sharing option.

Procedures: Minor procedures can be performed when there is a medical necessity, but availability may vary.

Additional Costs: Some medications, tests, and procedures conducted at the clinic may come with extra charges. You will be informed of any costs beforehand.

Eligibility Restrictions: Individuals enrolled in Medi-Cal/Medicaid cannot join due to state regulations. Those on HMO or EPO plans may find that the membership offers limited benefits, as some networks require referrals to their providers for various services.

Medicare Concierge Plan: To provide a Medicare Concierge service, we bill Medicare for covered services. The concierge fee is meant for improved access to our healthcare providers and services but does not include the cost of the services themselves.

Membership Enrollment: Online sign-up is not available. To enroll as a member, you must do so in person at our clinic or via telephone.
